Y6-Y7 Transition Form for Music Lessons

We have now opened our transition form for any Y6 students having music lessons who would like to continue in Y7!

It's an exciting time getting ready for secondary school but we also know it can be stressful too for families. In order to make lesson transition as smooth as possible, you should find within your Eepos account your young musician's transition form. If you could complete this, then we can move your studies across with them to their new school. If, for any reason, we don’t deliver lessons in that school or lessons are delivered by either one of our fabulous accredited teachers or private provider, we will let you know ASAP. We will also be able to confirm with our colleagues in schools who is signed up for which instrument.

If you can’t find the Year 6 Transition form, the information regarding a change of school can still be added onto the 23/24 Renewal form. You can also email MusicHubOperations@sheffield.gov.uk if you are struggling to complete the form.
